Any of the games from the Silent Hill series. I can't really single out one as being the scariest.

I JUST finished reading Shadow Over Innsmouth, I'd love to see how they portrayed it in the game.
That's EXACTLY how it happens in the book, too.
Project Zero (just Zero in Japan, and Fatal Frame in the US). I have to play it with the lights on if I'm on my own. It's much creepier than Resi and Silent Hill (though the school in the 1st SH was pretty good).

Doom 3 is the scariest game i have ever played. The only game i have ever had to actually stop playing because i was afraid of what was going to come next.
Silent Hill was pretty scary, but in a different way to Doom... Silent Hill was more eerie than scary... Kept you wondering what was going on but never really made you jump, just made you want to pull the blanket closer up to your nose. Doom on the other hand is fucking nonstop pants shitting action...
Resident evil was never scary, just awesome. I can only really remember a couple of scary parts in each RE game... In RE1 there was the part when you return to the mansion from the guard house and you see the very first headhunter coming at you... Those things were scary simply because they could kill you in one hit even if you had full health just by swiping at your head.
RE2 has the biggest scare of any RE game in my opinion... When you go into that room with the mirror in it, pick up the jumper cable off of the table and as you walk out of the room a licker bursts through the window with the loudest smash... I swear i almost had a heart attack the first time i got to that part. Then there's the part in the second scenario where you walk along that corridor and get to a CCTV camera, and as you turn it on the T-90 is walking towards it, and the only way out is to get past him. Oh and when he smashes through the wall in that room where you have to light the 3 furnaces in a certain order to release a cog.
Damn, i want to play scary games now...

Silent Hill 2 is the only game I actually became too scared to continue playing for a while. That scene where James is in the closet and Pyramid Head fucking rapes that mannequin monster made me drop the game controller and switch over to see if Seinfeld was on.

Scariest game I every played is Fatal frame as well. I've never dared to play it by myself, only with people around and with a well lit room. That game is my worst nightmare, I never got too far and don't plan to ever play it anytime soon.
Second scariest was REmake. Man, that game was good. Scared the shit out of it, has fuckin awesome graphics and those crimson heads just scare the fuckin SHIT out of you. Good times, good times.

The only part of CONDEMNED that scared me was when you were crossing the rope, near the escalators, and then the camera turns you and BOOM, ugly man in your face. I so wasn't expecting that. I think 2 drops of pee came out.

Ahh. Finally someone else who thinks Fatal Frame/Project Zero is scary. The first time I played it, I was on my own in the dark in my bedroom. I wasn't expecting it to be scary. In the end, I turned it off and only played it about two more times after that. It was very scary.
And REmake is a very good game, which I've yet to complete. I love getting scared, but it's stopped me from completing the game.
But how about the Regenerators from RE4?:ahhh:

those crimson heads or whomevers from RE1 gamecube remake. Especially in the one room that had a short, maze like hallway and a mirror, and its been awhile since you remembered that they come back to life, and you see a flash in the mirror then some fast little bitch biting you on the face.
That was awesome.

Crimsons are the worst. I have 2 shit my pants stories from those damn things...
1. It was in that room where there's a bed and a closet. You go to the desk and the closet opens and the zombie walks out. You kill it and read the diary, it's talking about how they were making experiments and he had to wear a suit. He got infected and ate his friend. Well, I left that room without thinking and played the game on. Whilest looking for where to go I stumbled upon that room not having any recollection of what I did earlier. As I walked around the bed, the camera view quickly shifted and to dear god that son of a bitch, out of my sight the whole time, jumped up bleeding red and screamed for blood. I nearly shat myself, fuck that was unexpected. And....
2. It's the hallway that is like a big u, and there's a staircase. Usually there are 2 or 3 zombies walkin around this hallway and I popped 'em all. Think I lit up 1 or 2 of them and the other, once again because of a beautifull camera angle the zombie was hidden from my view. So of course I came back to that area later on not knowing it was there and than out of no where a crimson head jumps out and screams while his acid breath smokes up the air.
Scariest moments in a game I've ever experienced.

Oh I know - you're alone in a dark locker room with a bloody body stuffed into a locker, and they want you to take a picture of his face? I immediately thought "this fucker's still alive, he's going to jump me", and of course, he did, and I still crapped myself.

The original Silent Hill was super creepy and well made. The bit near the beginning when you're running after your daughter and you can only just make her out in the fog; that's so well done considering the graphical capabilities of the PS1. Also, running around the School and Hospital in the dark was scary.
In RE4, when you're Ashley and you have to walk down those almost pitch-black passages with those suits-of-armour on either side, that was hard to play.
About 15 years ago on the Commadore 64, or something like that, there was a game which started off with a cut-scene where your baby is stolen by a big winged demon thing, during a storm, and that gave me nightmares and was too scary for me, but I was only 6 or so.
I personally think games can be way scarier than films, if they're done right.

F.E.A.R and Condemned Criminal Origins, played in the dark whilst stoned, scared the fuckin shit out of me, i really get into things and CCO scared the fuckin shit out of me plenty of times, i was such a pussy playing that game in the dark, my imagination runs wild and that serial killer fucker always made me jump, game was fairly easy though completed it quickly on hard, between heart attacks and cleaning my shitty pants. F.E.A.R didnt scare me as much but it did have its scary points.
Had to edit because i thought of how my friend is scared by even Rainbow Six Vegas, i got him to play it and he near shit his pants when a guy shot at him, "WOAH" he kicked my desk nerly fell out of the chair, panicing he pushed all the buttons runs into a wall, then reverses still gettin shot at and backs into a corner spraying at dust particles then dies, he then quit. But this is the guy that as a kid also would turn his playstation off when he see the first zombie in resident evil, he would literally be scared to the bone when he see the shadow of the zombie moving and hearing the moaning get closer, he would turn it off or close his eyes until he heard the death music, took him a very long time to complete it.

Clock Tower 3
THANK YOU!
Clock Tower 3.
A 15-year-old girl (you) getting chased every level by serial killers. Now that's scary. And when you use the hiding spots (like behind the curtain) and you can see the killer just on the other side of the curtain...and your panic meter starts to rise and is less than an inch away from Panic Mode (which causes your character to burst from her hiding spot)...
Man, that is intense. It doesn't help when they can teleport and appear out of nowhere (behind walls, doors, etc.) and taunt you with in their deep, gravely voices.
Scary shit.

man, nobody mentioned eternal darkness? the game that would say your controller is unplugged in mid fight, when it wasnt, it would act like your tv shut off, and omg once it said my memory got erased, greatest mindfuck game ever made.
Eternal darkness is the GREATEST mindfuck game ever. I very much agree. Too bad so many people overlooked it, I loved that game. I would let myself go insane just to see the fucked up affects.
For those who haven't played, you play as a bunch of different characters in time and you are all from the same family. The main character sees that families past or reads about it as you play it. The part of the game that fucks you is the insanity meter. Your meter depletes as monsters look you in the eyes and once it lowers you get FUCKED UP. The game will fuck you over. Not only will crazy shit happen in the game, but it'll say fucked up things about things on the outside. Such as you entering a room and it says your controller was unplugged as you see a swarm of monsters kicking your ass. Or the volume will go down or the tv will shut off. Other things include your body parts falling off, your head exploding, the room getting smaller, and many many more.
Man does it get you, it's so awesome how they did it. I played with my friend and the volume went down all the sudden, I was like "Dude, stop being a pussy and turn the volume up". So I turn around to look at him and he gives me this look, the remote nowhere near him. I was like.... "Oh, this game is good..." XD Love it, check it out.

Silent Hill 2 was probably the most fucked up game I've ever played. I don't really know what constitutes a scary game. The jumpiest one was definately RE2, with that dude in the olive green trenchcoat bursting out of the fucking wall every 8 seconds.
A scary game is one that fills you with fear or dread.
Silent Hill 2 wasn't as creepy or jumpy as Silent Hill 1 and if you thought that was scary, or that Clock Tower sounds good... really try out Zero/Fatal Frame, I'm a 30 something man, and I still need all the lights on.

You guys arent alone. The fucking giant things in the hedge maze near the forest temple scared the shit out of me back in the day.
So I'm just walking along, minding my own buiseness, when this giant brown dude comes out from the fucking corner and goes "BLAAARRRHOOAHHHRRRGGAAAARRGGGGNIGGA!"
And then he sodomizes me with his spear and throws me into the pond.

FFXI for real. I'm going past valkurm dunes to get to selbina to finish my subjob quest, and what do you know! A goblin tinkerer started following me. I had to zone quick because I only had 5 hp left. The part that scared me is that my home point was in bastok and I'd have to walk for an hour an a half to get back. I made it but I nearly shit myself.

Zelda shadow temple! I must have been 7-9 at the time...the 2 pictures a few replies above me show mostly what I was afraid of. Those monsters with the 4 hands that came out of the ground and if you got close they grabbed you on the head and the big beast controlling them came and bit you!! Well that and how the shadow temple was always so dark.

mine would be condemned criminal origins
all the bad guys are on crack and steroids and the only weapons you get are thigs you find lying around. ex a paper cutter blade.and bosses like the lunch lady. but the most interesting part was when I was teleported to this pool where the deep end is slightly filled with blood. its messed up
I just finished Condemned and fuck was that scary. Just brilliantly made. Especially the school part is some of the best I have ever played.
Those guys at monolith knows how to make a good horror game. They also made F.E.A.R

Maybe it's just because I was younger but I thought playing as a marine in the first pc Aliens vs. Predator was scary as shit.
But if I played doom3 or fear back then I might have thought they were scarier. Honestly I just finished up fear about a month ago and I didn't think it was that bad, the ending was definitely freakin awesome though.

the part that got to me was hardly being able to see anything and hearing the pinging noises on the radar. Then you would come to a tight room with plenty of placesfor the enemy to come out, and I'd be like "youve got to be kidding me you want to go in there"

Wow, I thought I was the only one who got creeped out playing OoT when they were a little kid. The wall masters scared the shit out of me when I got caught by one. Thanks to that I skipped the forest temple and went straight to the fire temple. Then when I got the courage back I was able to beat the game. But the wall masters, floor masters, the entire shadow temple/well (and everything in it), and those brown things in the forest maze creeped me out when I was a kid.
The scariest game for me would have to be F.E.A.R since things jump oyt at unexpected moments in the game when it ie quiet.
