I just want some confirmation on how it should exactly be filled out before i claim my prize. I'd appreciate only answers from people who have claimed and received prizes, or someone who definitely knows how it should be. I believe it would be like this.

And you received your prize? Also, I'm trying to claim the Fujifilm Finepix Digital Camera. I got the eReps for it, (332,000) but i was just unsure of the shipping address setup.
